Job Description

We are looking for a Care Manager who believes that great care changes lives. In this role, you will be at the heart of our at-home care services, guiding clients and families through their care journey, leading caregivers and nurses, and ensuring every client receives thoughtful, personalised, and compassionate support. 

As a Care Manager, you will oversee the full care journey, from understanding client needs and creating personalised Care Plans, to coordinating caregivers and ensuring service excellence. You will work closely with clients, families, caregivers, and medical teams to deliver care that is professional, reliable, and deeply human.

You will also play a role in supporting care package sales, daily operations, and continuous improvement of our care services:

  1. Supervise Care Delivery Process:
    1. Oversee the overall care provided to our clients.
    2. Ensure high-quality service delivery.
  2. Sales and Client Management:
    1. Win deals with clients and handle sales.
    2. Maintain regular communication with clients.
  3. Care Pros Management (Assigned Caregivers):
    1. Supervise assigned caregivers and nurses.
    2. Select and assign caregivers for specific cases.
    3. Assist with onboarding new caregivers.
  4. Client Relationship and Communication:
    1. Develop and maintain effective communication channels with clients and their families.
    2. Assist patients in scheduling appointments and organizing concierge services.
  5. Care Plan Development:
    1. Collaborate with the medical team to create and update individualized CARE Plans for clients.
    2. Adapt plans as needed based on changing conditions.
  6. Meeting Participation and Reporting:
    1. Attend weekly homecare meetings.
    2. File daily reports on cases in the system. 
  7. Financial Responsibilities:
    1. Create invoices and quotations for all cases.
    2. Report back to the Homecare manager on financial matters.


Job Requirements

  1.  Background in healthcare, nursing, allied health, or care services (preferred).
  2. Experience in home care, senior care, or service coordination is an advantage.
  3. Comfortable working with clients, families, and frontline care teams.
  4. Able to manage multiple cases with responsibility and attention to detail.
  5. Professional, dependable, and committed to delivering quality care.
  6. A positive attitude, willingness to work and learn new skills
  7. Having an open mindset & constantly learning new knowledge
  8. Works well within the team
  9. Initiative, pro-active & committed to the jobs 
  10. Hardworking, work smartly & able to meet any deadlines given
